The new Dementia Advice and support service for people living with Dementia and family carers in Sheffield, aims to provide advice information and support from diagnosis onwards.
The service aims to:
• help to keep doing what you’ve always enjoyed doing
• a listening ear
• Dementia information
• support from others in a similar situation
• groups and activities
When you get a diagnosis of Dementia
A couple of weeks after your diagnosis, you will be referred to this service and you will receive a phone call to identify your unmet needs/interests and to inform you of the groups and activities that you may be interested in, a referral to the local Alzheimer’s Society Dementia Support Worker and your local Memory Cafe.
After the initial call
You will receive a 6 monthly call to find out if anything has changed or you need more support/information.
In Between calls
If an issue arises or you need information, the contact details are below for your local area, followed by some national organisations.
